Fill out all required services and include delivery and payment information and one form for each pair of shoes.
STEP ONE:
- Select whether your shoes are Ladies or Men’s
STEP TWO:

- Complete the form including services/shipping/billing information.
- Print the form and place the form inside one of the shoes.
- Please keep a second form for your records.
STEP THREE:
- We recommend using a Tracked or Registered delivery service such as (An Post)
